At the moment, Cochrane is still focused on the work of the Review Groups, which are responsible for producing the systematic reviews that are then disseminated through the Cochrane Library and other traditional forms of publication. Field groups, on the other hand, are responsible for coordinating the work of review groups, raising awareness of the need for and importance of systematic reviews, and promoting the preparation and maintenance of specialized databases. Geographical centers, such as Cochrane Italy, are responsible for disseminating Cochrane values and practices in different languages, providing support and a space for collaboration for centers in the field.

Cochrane Drugs and Alcohol Review Group (CDAG) is the first group to have its editorial base in Italy, starting its activities in 1998. CDAG produces, updates and disseminates systematic reviews on the prevention, treatment and rehabilitation of problematic drug and alcohol use. It has its editorial base in Rome at the Department of Epidemiology of the ASL RE, Local Health Authority of the Lazio Region.

Cochrane MS and Rare Diseases of the CNS Review Group coordinates the preparation and production of systematic reviews aimed at covering all aspects of the diagnosis, prevention and treatment of multiple sclerosis, as well as rehabilitation and palliative care for people with multiple sclerosis and other inflammatory demyelinating conditions of the central nervous system.

Cochrane Global Mental Health (CGMH) Field works with authors from around the world to collect and summarize scientific evidence on care for people with mental health problems living in low- and middle-income countries (LMICs). They also support the dissemination, implementation and use of this knowledge. CGMH provides input and training on evidence synthesis and implementation activities related to mental health in LMICs. It aims to develop a mutual learning approach and exchange of knowledge between mental health workers in different parts of the world.
